This form can be used for:
1) a one time special request or appointment,
2) a request for final exams, or
3) cancellation of a captioning service
Any requests for captioning services for academic classes must be submitted on the Request for Captioning Course Schedule form.
In order to ensure that Captioning Services are provided, please allow at least 72 hours advance notice for requests. Requests with less than 72 hours notice cannot be guaranteed.
