Who is Considered a Qualified Student with a Disability?
In order to provide quality services to students with disabilities, Texas State University-San Marcos, requires a student provide verification of disability to the Office of Disability Services (ODS), at the time services are requested. A student who has an existing diagnosis or documentation of Attention Deficit/Hyperactivity Disorder, inattentive, hyperactive or combined type (AD/HD) which has been completed by a certified professional (e.g., psychologist, psychiatrist or physician) within the last five years may submit this documentation to the ODS for consideration. In certain cases, updated information may be required. This information will be used to establish the need for appropriate accommodations.

A student needing documentation of AD/HD can receive referral from the ODS. It is the responsibility of the student, not the ODS or the university to pay for the cost of an evaluation.

Students should keep a copy of any disability documentation provided to the ODS for their own records. The ODS will maintain a student's file no longer than ten years after graduation or the last semester services are requested.

What is AD/HD Disability Criteria?
The characteristics of AD/HD may be grouped into three domains; inattention, hyperactivity and combined type. Each student with AD/HD has a unique set of traits and characteristics. The following list of characteristics may be associated with AD/HD: inattention, distractibility, impulsivity, activity level or arousal problems.

I. Required Documentation
A. Evaluation completed by a psychiatrist, psychologist, or neuropsychologist that includes:
1. Developmental history – including evidence of early and current impairment
2. Types of assessments used in the evaluation
3. A medical or clinical diagnosis that meets criteria for the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV-TR) diagnosis of AD/HD as identified by the ODS Verification Form for the Diagnosis of AD/HD
4. Quantitative and/or qualitative information that supports the diagnosis
5. Area impacted by AD/HD and severity of the condition (past and present)
6. Recommendation for accommodations including the rationale for the recommendation

II. Recommended Documentation
A. Intellectual Assessment – recommended tests include: Weschler Adult Intelligence Scale-Third Edition  (WAIS-III), Woodcock Johnson Tests of Cognitive Ability
B. Achievement testing – recommended tests include: Woodcock Psycho-Educational Battery-III
C. Medical evaluation noting medication prescribed, if any, and its effects and side effects
D. AD/HD behavioral rating scales
E. A continuous performance measure of attention and response control (e.g., Conners Continuous Performance Test, IVA, etc.)

How are Accommodations and Support Services Determined?
Within 30 days of receiving the diagnostic evaluation or report, the ODS will review the need for accommodations and support services based on the individual's disability documentation and disability-based need.  The ODS will notify the student after determining appropriate accommodations in accordance with Section 504 of the 1973 Rehabilitation Act, the 1990 Americans with Disabilities Act and university policy.  For a description of services available for students with disabilities at Texas State, see the Office of Disability Services brochure.
